Athletic Sign-up Procedures

No Athlete Will Be Allowed To Partcipate Until All Procedures Are Followed
Athletic Department Permission Form - Form must be completed, signed and returned to Athletic Director's Office

Health History Form - Form must be completed for each season, signed and given to nurse. The nurse will give the student a clearance form to give to the coach.

Health Physical - Updated physical must be on file in nurse's office. The nurse will give the student a verification form to give to the coach.

Authorization For Emergency Treatment Of Minors Form - Form must be completed and given to the coach.

Payment to the Athletic Director - Student will receive a verification of payment receipt which will then be given to the coach by the student.
