The school nurse assesses students for illness, provides first aid, administers medications, and provides vision, hearing, andHealth Office spine screenings. She also coordinates medical eligibility for students’ participation in physical education class and school sports. Please contact the school nurse if you have special concerns about your child.

Emergency Contact and Health Info

We require current parent/guardian phone numbers on file in the health office in the event of student illness, injury, or emergency. We ask that this information be updated annually on a student emergency contact and health information card. Students receive this card from their homeroom teacher during the first week of school and are asked to return the card to either their homeroom teacher or the nurse before the end of September. Please notify the guidance department or health office of changes throughout the school year.

Illness/Injury

Students are encouraged to see the school nurse when they become ill at school and to seek first aid in the health office for injuries that occur during the school day. No student should leave the building because of an injury or ill health without first being seen by the nurse and the nurse having permission for the student to leave the school from their parent/guardian. The student will then sign out at the attendance center once permission has been given.

Medication Use in School

When it is necessary for your child to take medication at school, either OTC or prescription, your child’s health care provider must complete our medication order and authorization form in addition to providing parent/guardian signature authorizing the nurse to administer the medication. All medication must be stored in the health office and administered by the school nurse. An exception can be made for an Epi Pen or inhaler when an additional authorization is on file in the health office. This “carry” authorization is mandatory for athletes whose health care provider prescribed an Epi Pen for a bee sting allergy or an inhaler for asthma. **

Annual Screenings and Health Appraisals (Physical Examinations)

Per regulations of the NYS Commissioner of Health, health appraisals are required for all 7th and 10th graders and students transferring in from other districts. Annual screenings are required for students in grades 7 through 10. Students in grades 7, 8, and 9 require scoliosis screening and those in grades 7 and 10 require a hearing and vision check. The school nurse performs these screenings on all students who do not show documentation of these having been performed by their health care provider. You will be notified should the results of the screenings indicate that your child requires further follow-up. If your child has had a health appraisal by his or her own provider please forward the completed appraisal form along with an updated immunization record to the health office.  Having this information on file will help to avoid duplicating the screenings already performed. If your health care provider has not examined your child and you do not want your child screened for scoliosis by the school nurse, notify the health office in writing prior to October 3rd.

Athletics

The medical requirements for participation in school sports are met when the following papers are on file in the health office:

  • A health appraisal that specifically notes sports participation (had the examination within the last twelve months)
  • A health history that has been completed and signed by a parent/guardian(a new one is required before each sport played)
  • Documentation of a tetanus booster given within the last 10 years
  • A release from your child's health care provider to participate after an injury, a surgical procedure, or a chronic medical condition.

Written permission and completion of a health history form by a parent/guardian are required before the school physician will examine your child. 

Health Requirements for Newly Accepted Students

 All accepted students are required to have the following documents on file in the health office prior to starting school. These documents are reviewed by the school nurse to establish that all NYS immunization requirements are   met and that your child’s health issues are understood so that pertinent information can be disseminated to the appropriate school personnel.  At BGP, we make every effort to see that your child’s school health needs are addressed to ensure a healthy and positive school experience.

  • An Official Immunization Record (parent should obtain from health care provider)
    • This official document is generated by your child’s health care provider (hcp). It should be legible and written in English.
  • A Health Appraisal ( to be completed by your child’s hcp)
    • For enrollment purposes, the physical exam can be dated no earlier than September of the school year just completed. Once school begins a health appraisal is required for the current school year.
    • Foreign students must also have a physical examination completed by our school physician or a private NYS physician by November 1st.
  • A Health History (to be completed by parent)
  • Medication Order/Authorization (to be completed by hcp and parent)
    • This is left to parental discretion, but is required if your child will be taking prescription or over the counter medication during the school day or has been prescribed emergency use medications. (i.e. Epi Pen, benedryl, inhaler, glucagon)
  • Dental Health Certificate (to be completed by your child’s dentist and parent)
    • This is optional but recommended.
