Dear Parents,
New York State Education Law stipulates that all athletes must have an annual medical examination. Prior to the start of each new sports season, the student must individually see the nurse for clearance. A signed athletic form is to be submitted only to the nurse. Coaches will not accept forms from student or parent. It is the parent’s responsibility to keep track of this and the student’s responsibility to come and check with the nurse if he is not sure of dates. In addition, if your teen has had an injury or had surgery since his/her last sport clearance, we need written clearance from the doctor for him/her to participate in a subsequent sport. Thank you!
